Co-ordinator - Dementia Services (OZCARE2406)

Overview

The Co-ordinator - Dementia Services will be responsible for the provision of dementia services funded by HACC, to provide education, information and support to HACC service providers. In addition the position will undertake specialist consultation and co-ordination to HACC eligible clients and their carers to improve the quality of life for people effected by Dementia. The successful applicant will have broad experience in the delivery and co-ordination of dementia services and be willing to work in partnership with a wide range of key stakeholders.
