If you are living with dementia and need some assistance to stay in your home longer, or are a carer who would benefit from respite care, we can help you with a complete suite of home care services.

Our home care services are delivered by our highly skilled, friendly staff who travel around the community every day providing care services. They are experienced in caring for people living with dementia and will tailor your care plan to your specific needs.

We work with you, your carer, and your family to improve your independence and stay living at home, surrounded by your loved ones, for as long as possible.

Home care services for people living with dementia

    Including showering, dressing, eating, and other everyday tasks
  • Meal preparation
    For nutritional, tasty food
  • Domestic assistance
    General housework including cleaning, ironing, and shopping
  • Transportation
    Around your community or to your appointments
  • Social support
    Helping you maintain an active social life in and outside your home
  • Nursing
    Specialist care in your home for wounds, continence, medication, chronic disease, and palliative care
  • Allied health
    Including physiotherapy, dietetics, social work, and occupational therapy

Home care services for carers

  • Respite care
    Available both overnight and during the day in your home. We also offer day respite centres and residential respite care

Our approach to home care

Our home care services help people to maintain or re-establish daily living skills and community connections to stay living at home longer.

To achieve this, we focus on helping you to regain skills and increase your independence for the activities of daily living including moving around your home, personal care such as showering and dressing, food and nutrition, health and wellbeing, social connections, and travelling around your local community.

Our team of skilled dementia specialists, nurses, care workers, physiotherapists, occupational therapists, dietitians, social workers, and exercise physiologists work with you and your family to create a support plan that takes your individual situation and goals into account, to provide personalised care tailored to your specific needs.


Availability

Our home care services are available to people living with dementia and carers in Queensland, as well as a variety of other people including seniors and those living with a disability. To find out which services are available in your area, use our locate a service feature.

Costs

The cost of your services is based on your income and your eligibility for government-funded subsidies. Depending on your age, the Australian Government or Queensland Government will do an assessment to determine this, and advise if you will need to contribute towards the cost of your care. Home care packages

Any of our services, including our range of dementia services, can be delivered as a home care package. Home care packages are available to seniors and, in special cases, for those living with a disability. You will need an Aged Care Assessment Team (ACAT) assessment to access a home care package.
