Assistance with Care and Housing for the Aged
The Assistance with Care and Housing for the Aged (ACHA) Program provides assistance to frail, financially disadvantaged, older people or those with a disability, who are renting, in insecure accommodation or are homeless.The focus of the program is to assist eligible people with their accommodation and support needs, allowing them to remain living in their community.
The program operates across Brisbane North, Brisbane South (including Redlands) and the Sunshine Coast.
Support staff, using a case management model, may provide assistance in a range of services including:
Identification of client care needs and support services
- Discussing possible care and housing options.
- Referral to Aged Care Assessment Team (ACAT), Home and Community Care Services (HACC) and other health related services.
Linking clients to housing services
- Assisting clients to find suitable accommodation.
- Assisting clients to access financial or legal assistance (rent relief, bond assistance, tenancy advice).
- Assisting clients to access services which would improve their current accommodation.
- Assistance with building local networks of social and service agency contacts (including ethnic or Aboriginal and Torres Strait Islander communities where appropriate).
- Organising utilisation of services provided by other agencies, including both voluntary and Government.
- Assisting clients to utilise their own resources.
- Identifying barriers to service delivery amongst care and housing agencies, and working with these agencies to overcome the barriers.
