Residential Support Program

This Program assists people who are homeless or at risk of being homeless, including men and women who are living in hostels, boarding houses, public housing or caravan parks. 

The aim of the program is to identify the health and social needs of the frail aged, people with a disability and people experiencing difficulty associated with mental health and/or drug and alcohol abuse.  

The Program provides access to mainstream services for a group of people who traditionally do not access services.  By providing a flexible approach to service delivery, the Program encourages clients to address their social, emotional and health needs.

Staff work with eligible clients to identify support and services which may be of assistance. 

Through educating clients about available services within their community and by encouraging self-advocacy, clients will be better equipped to independently access these services in the future.

Services which can be accessed through the program include -

  • Community Care - including personal care, domestic assistance, social support, transport for health and allied health appointments
  • Community Nursing - including wound and medication management, continence management, diabetes education and health screening
  • Allied Health Services - including assistance with podiatry, physiotherapy, occupational therapy

This Program offers a flexible model aimed at enhancing the quality of life for the homeless or those at risk of homelessness who have a variety of health and social issues such as:

  • Medication management and education
  • Assistance with personal care
  • Social isolation
  • Difficulty accessing Doctors, Dentists, Allied Health     Professionals and Mental Health Services
  • Lack of information on health and preventative health services
  • Drug and alcohol issues

Referrals can be made through -

  • Self-referral
  • Health clinics at local Homeless Hostels and Drop-In Meals Centres
  • Hostel, boarding house and caravan park owners/supervisors
  • Carers, family members or friends
  • Hospitals / Doctors / Emergency Services / ACAT
  • Allied Health Professionals or Local Service Providers
