Extended Aged Care at Home Packages - Dementia

Extended Aged Care at Home (EACH) - Dementia Packages provide a high level of care to people with Dementia and associated behavioural conditions.

The range of services includes:

Personal Care (showering, grooming, assistance with daily living activities)
Nursing and Allied Health Care
Meal Preparation
Respite
Transport to Medical and Health Appointments and Shopping
Assistance with Specialised Equipment

Aged Care Assessment Team (ACAT) approval is required.
