Dementia Services
The Program assists people living with dementia, their carers and their service providers.
The service can help by:
• Providing information and more understanding about dementia.
• Helping people to find available community resources.
• Directly referring to specialist services if required.
• Assisting HACC funded service providers with:
- Support in identifying dementia-specific and related client needs.
- Information about dementia and available resources.
