Our home care services are delivered by our experienced, friendly staff who travel around the community every day providing care and support services. Our staff genuinely care about their clients and spend time getting to know you; your history, your family, your likes and dislikes, to ensure we’re delivering the best care possible. We make sure that you continue to see familiar faces for each of your appointments so you can get to know your carers and feel comfortable with them.
We work with you, your carer, and your family to improve your independence and stay living at home, surrounded by your loved ones, for as long as possible.
Our home care services are available to people living with dementia and carers in Queensland, as well as a variety of other people including seniors and those living with a disability. To find out which services are available in your area, use our locate a service feature.
The cost of your services is based on your income and your eligibility for government-funded subsidies. Depending on your age, the Australian Government or Queensland Government will do an assessment to determine this, and advise if you will need to contribute towards the cost of your care. Any of our home care services, including our range of dementia services, can be delivered as a home care package. You will need an Aged Care Assessment Team (ACAT) assessment to access a home care package. Read more about home care packages.
We believe people can live well with dementia with the right support and services. We help you to live at home for as long as possible by building your capacity to complete daily activities independently, focusing on your strengths and personal goals.
Our home care services help people to maintain or re-establish daily living skills and community connections to stay living at home longer.
To achieve this, we focus on helping you to regain skills and increase your independence for the activities of daily living including moving around your home, personal care such as showering and dressing, food and nutrition, health and wellbeing, social connections, and travelling around your local community.
Our team of skilled dementia specialists, nurses, care workers, physiotherapists, occupational therapists, dietitians, social workers, and exercise physiologists work with you and your family to create a support plan that takes your individual situation and goals into account, to provide personalised care tailored to your specific needs.
