Maintain an active social life in your community

Being an active part of your local community and keeping in touch with friends and family can be more difficult to do if you aren’t able to get out of the house because of a disability or health condition.

We offer a range of social support services to help you maintain an active social life, including

  • Visits to your home to spend time together, sharing your hobbies and interests
  • Helping you shop for food or other things
  • Helping you find groups with similar interests such as a support group or hobby group
  • Visiting friends or family

Costs & Eligibility

Our disability and health services are available to people under the age of 65 (or under 50 years for Aboriginal and Torres Strait Islanders), who have a disability or other condition that makes it difficult to carry out everyday activities.

The cost of your services is determined by your eligibility for Queensland Community Care Services (QCCS) funding. If you’re not eligible for subsidised services, you can still receive services by paying the full cost yourself. Read more about costs & eligibility for people with disability or health conditions.

Our Approach to Disability Home care

With 20 years’ of experience working with people with disability and health conditions, our team of carers, nurses, physiotherapists, and other allied health professionals offer a complete suite of services to help you live independently at home. We assist you with a range of daily living skills focusing on your strengths and goals to create a personalised care plan that suits you.
